Leaders in Long-Term Care

Rehabilitate independent living skills

Whatever your medical condition, the Aultman Alliance Long-Term Care program is designed to ease the transition back into an independent lifestyle. From basic skills like getting in and out of a car to grocery shopping, so patients can return home as soon as possible.

The Community Care Center at Aultman Alliance Community Hospital is an extended-care facility with a family environment.

Caring staff members plan social and recreational activities for patients throughout the week.

Friendly, caring staff members go above and beyond physical care. Planetree ideals are implemented in daily activities, such as:

  • aviaries
  • children's visits
  • church services
  • spiritual resources

The community care center is a 78-bed extended-care facility designed to provide individualized interdisciplinary care, no matter the patient's needs.

The Center offers three levels of care – traditional, skilled and intermediate. Many of our patients have specialized care needs including:

  • rehabilitation services
  • complex medical care
  • convalescent care
  • end-of-life care

The Community Care Center is a department of Aultman Alliance Community Hospital that provides residents with the added convenience of a pharmacy, lab, radiology and emergency services.
