Palliative care is provided by a team of doctors, nurses, social workers, dietitians, chaplains and other specialists. They work together with a patient's primary care doctor or specialist to provide an additional layer of support in managing chronic or life-limiting illnesses. The goal of palliative care is to promote improved quality of life for both the patient and their family.

In most cases, palliative care is started in the hospital. The process begins with a referral from your doctor. Palliative care can also be provided outside the hospital where accessible in outpatient clinics, homes, hospices and skilled nursing/long term care facilities.
